I hereby certify that I am adequately fit to participate in this event.  In consideration or the acceptance of this enry, I, the undersigned for myself, my personal representative, beneficiaries, and heirs, knowlingly waive, release, and discharge any and all rights and claims which I have or may have hereafter accrue to me or my estate against the St. Anthony Catholic Church Youth Group, Brawt Trot or St. Anthony Catholic Church, its officers, directors, agents, representatives and the volunteers, sponsors, promoters, and all other persons associated for any and all injuries or death suffered by me in the event.  I HEREBY ASSUME FULL RESPONSIBILITY FOR ANY INJURY OR ACCIDENT WHICH MAY OCCUR DURING MY (OR MY FAMILY MEMBER'S) ATTENDANCE AT OR PARTICIPATION IN THE EVENT.
